TACKLE Referral Form

TACKLE is a non-profit behavioral health agency. TACKLE helps students who have referrals for a variety of mental health issues which can lead to poor attendance, behavior and academic performance. There is a case manager here that will meet with students on a regular basis and a therapist that can use utilized if necessary. Our case manager is Haley Harris.

Fill out this referral form and return to the counseling office if you would like to talk with Miss Haley about any mental health concerns.
